Fault Code I/O ERR on CPU — Channel Output Frozen at Last Value
This is the most common failure mode. The DAC on one or more channels locks up, holding the last commanded value. Your PLC program thinks it’s writing new setpoints, but the physical output doesn’t move. Check the I/O map in Unity Pro — if the channel status word shows 0x0002 (hardware fault), the card is gone. No firmware fix. Replace the module.

Replacement Procedure — Step by Step

1. De-energize the rack. The 140ACO13000 is NOT hot-swappable in standard Quantum configurations unless your rack is running in hot-standby mode with a redundant I/O drop. Confirm your architecture before pulling the card live.
2. Document the slot address. The Quantum backplane uses geographic addressing — the slot number IS the I/O address. Slot 3 in rack 1 is always Drop 1, Slot 3. No DIP switches, no rotary encoders. The address is physical. Seat the replacement in the exact same slot.
