1. Gap Voltage Out of Range (most common)
Your 3500/42M or CON021 is alarming on gap voltage — typically reading above –2 V (probe too far) or below –18 V (probe too close, or shorted). Before condemning the transducer, check the extension cable first. The PR9268 cable jacket is polyurethane and degrades at compression fittings exposed to steam or oil. Ohm out the center conductor and shield separately. If resistance is nominal but gap voltage is still wrong, the probe tip coil has failed — replace the PR9268/201-000 assembly.
