Common Fault Codes & Root Causes:

• DO_ERR / Channel Fault LED solid red: Single-channel transistor failure, typically caused by inductive load kickback from solenoids without flyback diodes. Before swapping the module, verify the field wiring — a shorted actuator will kill the replacement just as fast.
• Bus Communication Fault (BUS_ERR on MPC CPU): The DO232 has lost backplane communication. Reseat the module first. If the fault persists after reseating, the backplane connector pins on the module or rack may be oxidized or mechanically damaged. Inspect under magnification before condemning the module.
• All outputs dead, no LED activity: Check the 24 V DC field supply to the module’s load voltage terminals (L+ / M). The DO232 requires a separate field power feed — loss of this supply kills all outputs without necessarily triggering a CPU alarm.

Replacement Procedure (Field-Tested):
