After ten years of field work on rotating machinery, here is what actually goes wrong with PR6426 probe chains — and what to check before you condemn the module:

Fault Code: GAP voltage out of range (typically < -2V or > -18V on the monitor)
First suspect is probe-to-shaft gap. Before pulling the probe, verify the gap with a feeler gauge or dial indicator. The nominal gap for a 10 mm probe is 1.0–1.5 mm (gap voltage should read approximately -10 to -12 VDC at midrange). If gap is correct and voltage is still rail-high or rail-low, the probe or cable is open/shorted. Disconnect the coax at the CON021 input and measure probe impedance — an open probe reads infinite resistance; a shorted cable reads near zero.

Fault Code: High vibration / noise on the monitor with no mechanical cause
Check the coax connection at both ends — the BNC or threaded coaxial connector at the CON021 input is the most common failure point in high-vibration environments.   • Bypass/inhibit before work: Always place the monitor channel in bypass mode before disconnecting the probe. Pulling a live probe connection on a 3500 rack will trigger a latched danger alarm and may initiate a machine trip if the relay logic is not inhibited.

Common failure modes in service: Probe tip contamination with conductive debris (metal swarf, oil with metalite additives) causes apparent gap reduction. Cable jacket cracking from thermal cycling in high-temperature installations leads to moisture ingress and intermittent faults. CON021 driver failure is less common but occurs after 10+ years in high-ambient-temperature junction boxes — the oscillator IC degrades and output sensitivity drifts low.
