Fault 1 — No output after E-stop reset: Nine times out of ten this is a cross-short on the input channel wiring, not a dead relay. Before condemning the module, measure resistance between S11/S12 and S21/S22 with the E-stop button released. Anything below 500 Ω on either channel means your button contacts are welded or your cable has a ground fault. Replace the SRB402EM only after ruling this out — otherwise you’ll burn through a second unit in 20 minutes.

Fault 2 — LED status: input channel fault (red LED on CH1 or CH2): The SRB402EM monitors for symmetry between its two input channels. A timing discrepancy greater than ~500 ms between channel 1 and channel 2 opening/closing will latch the module into fault.
